Make sure you have the latest Recovery for the apexqtmo (I prefer TWRP)
Under Wipe:
Swipe to Factory Reset
Under advanced wipe checkbox:
Dalvik Cache
System
Cache
Data
Then swipe to wipe
Flash Rom And Gapps under install then reboot to profit
