i337 warranty statement copy? topic

My S4 stopped working yesterday. Luckily, I bought it with an Amex card, and it is just under 2 years since I bought the phone. I called Amex and filed a claim. Today, they emailed me and asked me to send a copy of the U.S. warranty statement and card that came with the phone. I searhed around my home but could not find the box and warranty statement anywhere..

AT&T website has one but it is watermarked and cannot be used :(

Could someone with the warranty statement and card forward me a scanned copy? I will be very grateful...
